York Region is now able to vaccinate residents 80+

Immunization is the best way to protect yourself and others against COVID-19

York Region is moving forward under their COVID-19 Mass Immunization Plan to offer vaccines to residents 80 years of age and older. Effective Monday, March 1, 2021 York Region residents age 80+ (born in 1941 or earlier) can book an appointment for their vaccination online by visiting york.ca/COVID19Vaccine.

  • Vaccines will be available by appointment only and only for York Region residents.
  • Residents age 80+ who are not able or not comfortable booking an online appointment are encouraged to seek out a support person (caregiver, family member or friend) who can assist in booking this appointment on your behalf
  • For additional assistance with booking appointments please call 1-877-464-9675.

This is great news for many of our most vulnerable residents and another step forward to bring an end to this pandemic through vaccination. Visit york.ca/COVID19vaccine for more information.


Coming Soon: CARF International to Visit CHATS

A survey team from CARF International* will be visiting on April 7th to April 9th, 2021.

CHATS has invited CARF surveyors to evaluate how well we meet international standards for quality. The survey will tell us what we are doing well and ways we might improve. As a result of this survey, we may earn or continue accreditation.


Southlake Community OHT is recruiting for their Patient, Client and Caregiver Partnership Council

Overview of the role: 
As a part of the Southlake Community Ontario Health Team, a seven-member Patient, Client and Caregiver Partnership Council (PCCPC) will be established. The Patient, Client and Caregiver Partnership Council will serve as the voice of patients, residents, clients, family members and caregivers through the implementation of the Southlake Community Ontario Health Team (OHT). For ease, these types of individuals will be referred to in this document and at the Council as “patients and caregivers”, to align with the Ministry of Health’s OHT guidance documents. It is expected that the selected representatives will represent the best interest of patients and caregivers within the Southlake Community OHT.  
 
Level of commitment: 
The PCCPC will have seven members including two Co-Chairs who will be elected at the first meeting. The committee will meet monthly for 2 hours and the Co-Chairs will also serve on the Joint Executive Steering Committee (JESC - the primary governing body of our OHT) and meets bi-weekly for 1.5 hours

Client Experience Survey: Results and Action Plans

We are pleased to share the results of our most recent survey and the actions we took based on the feedback provided. 

About the Survey Process:

CHATS’ Client Experience Survey was conducted in April and May of 2020. We sampled clients/caregivers from all program areas and are thrilled to have increased the number of respondents from previous years. Last year, 377 people agreed to complete the survey.  Below, we provide highlights of the results.

Survey Results Highlights:

Overall, clients and caregivers reported that they look forward to the CHATS programs and services they receive with 95.5% of respondents saying they would recommend CHATS programs to others. The larger number of survey responses and better representation from across CHATS programs, provides us with better and more meaningful information to make improvements. We are thrilled that so many took the time to not only answer the survey questions, but to provide us with valuable comments/feedback that provide us with an even deeper understanding of both our strengths and opportunities for change.

CHATS partners with the City of Vaughan to provide snow clearing services to seniors

Tuesday November 2, 2020

The CHATS Snow Clearing Program is a paid service that connects individuals with  pre-approved service providers for winter snow clearance services. The cost for snow removal is paid for by the homeowner. Subsidies are available based on the completion of a financial assessment.

Services consist of driveway snow removal, walkway clearing to the front/main door and salting on walkways to the front/main door. This program is available to Vaughan residents aged 55 and older or with age-related illness, for up to a maximum of 100 residents served.

Ontario Announces New Western York Region Ontario Health Team

Media Release: July 23, 2020

On July 23, 2020 Ontario announced 5 new Ontario Health Teams to better connect care and support Ontario’s COVID-19 response. The Western York Region Ontario Health Team was officially announced bringing to three, the number of Ontario Health Teams in which CHATS is an integral partner.

Since January 2019, the Ontario Government launched its plan to implement Ontario Health Teams for a more integrated approach to improving access and services to patients and families.

As of Wednesday, March 11th, 2020 the World Health Organization (WHO) “has made the assessment that COVID-19 can be characterized as a pandemic. Describing the situation as a pandemic does not change WHO’s assessment of the threat posed by this virus. New Accreditation Standards for CHATS

CHATS is pleased to announce that we have received Three-Year Accreditation with CARF. This is the highest level of accreditation awarded by this international standards organization. In order to achieve this level, CHATS went  through a rigorous peer review process and was able to demonstrate to a team of surveyors, its commitment to offering programs and services that are measurable, accountable and of the highest quality.
